Vital Heroes: answer health questions by moving
A Meta Quest exergame for adult rehab. Patients throw, slice, draw a bow or swing a bat to answer health questions.

In one look
The problem
Rehab patients get the right advice in a format they drop. Leaflets are read once, exercise sheets get skipped and videos are watched sitting down.
What we built
Put the lesson and the movement in the same moment. On a standalone Meta Quest, patients answer health questions by moving: throw a ball in Cardio, slice with a light blade in Motion, shoot a bow in Breathe, swing a bat in Neuro.
Where it is now
A working Quest build in English and Bulgarian, and a live clinic website where a team can play one challenge and book a demo. It still runs on a lovable.app address, and it claims engagement, not clinical results.
What this would have cost in 2024
About $14k and 5–7 weeks
2024 team: 3 people: Unity VR developer, web developer, designer and health-content writer. Built here by one founder working with AI in 8 days + a 13-day refit.
| Piece | 2024 hours |
|---|---|
| Quest hub, tutorial and holographic organ models | ~53 |
| Four module games on the studio's quiz engine (ball, blade, bow, bat) | ~73 |
| 20 health-awareness challenges in EN and BG | ~26 |
| Music, narrated tutorial, promo video, logo | ~32 |
| Unity 6 port, new XR rig, Quest build and headset fixes | ~73 |
| Positioning research, claims check, site copy EN/BG | ~29 |
| Clinic website: 3D organ stage, playable challenge, demo form | ~72 |
| QA on headset and web viewports | ~19 |
| Total (PERT) | ~378 |
Method: PERT (O+4M+P)/6 per piece; low = O × $25, likely = PERT × $37.5, high = P × $50 (Accelerance 2024 Eastern-Europe band). These are estimates of what a team would have needed in 2024, not invoices.
How it was built
From first commit to current build in 8 days + a 13-day refit
- First working version
The VR Express studio commits a working Vital Heroes for Quest: an intro hub with floating holographic organs, a tutorial, and quiz games built on the studio's Meta Quizzes engine.
- Final February version
Music, fine-tuning and final changes across the four modules: ball, light blade, bow and bat, each asking questions on its own health topic.
- Ported to Unity 6
Ilian used Claude Code to carry the February build over to Unity 6.3, keeping its original scenes and assets, and set it up for Quest with OpenXR.
compiles with 0 errors in batch mode - Reference scene for a shared engine
Vital Heroes becomes the reference scene of the studio's rebuilt quiz engine, with questions stored as data instead of baked into the scene.
- Bulgarian
The four quizzes get Bulgarian content next to English.
- A standard XR rig
The old hand-physics player is retired for a standard XR Interaction Toolkit rig.
- Re-tested on a headset
Environment, teleport and pointer rays fixed and checked again on a Quest.
- Audience pinned to the build
Older pitches had drifted from what was built. Ilian chose adult rehab, sold to clinics, and the site's claims were checked against a list of verified facts.
- Clinic website live
A bilingual site built in Lovable: the four organ models as a 3D hologram stage, a challenge you can play in the browser, research drawn from outside studies, and a demo request form.

Who did what
The founder decided
- Adult rehab, sold to clinics: the audience the build actually serves.
- Pitch engagement, not outcomes. The research section cites outside studies and says they are not studies of Vital Heroes.
- Every asset carries the line: a health-awareness and engagement tool, not a medical device.
- Progress tracking is not claimed until it is built. The site says "not yet".
AI built and checked
- The Quest game itself is older: built by the VR Express studio in Unity in February 2026.
- Claude Code: the Unity 6 port, the new XR rig, the Bulgarian content and the headset fixes.
- The clinic website, built in Lovable from Ilian's briefs, with its copy checked against the verified claims.
- The groundwork: Claude read the pitch history, the Unity project and the research papers to settle what the site may say.
What's next
- Score export and a clinician view
